Your Consultation

At your initial appointment, Dr. Galen will perform a thorough examination, explain his diagnosis and propose treatment that you may need. Some surgical procedures can be preformed on the same day as the consultation. More extensive procedures may require an initial consultation and the surgical treatment is scheduled for a later date.

Minors (under the age of 18): A parent or legal guardian must be present at the time of consultation and on the day of the surgery.

Please alert our office upon making an appointment of any serious medical conditions that may be of concern prior to surgery (premedications necessary, any anticoagulation therapy that you may be receiving).

What To Bring To Your Appointment

On the day of your consultation, please have the following information with you:

·Referral request card (prescription) from your doctor

·Any x-rays that your doctor gave you to bring to the consultation or request that they are sent prior to your appointment. Additional x-rays may be required and taken at our center

·The name and dosage of Any medication you are taking

·Dental and medical insurance information

FINANICAL POLICY

In order to provide the finest care at the most reasonable cost to our patients, payment is expected at the time services are rendered. You are fully responsible for all fees charged regardless of your insurance coverage. We are not providers for any dental, medical or medicare insurance carriers. As a courtesy we will submit your insurance claim and reimbursement will be directed to the insured. For your convenience we accept Visa, MasterCard and Discover card.

SCHEDULING

Our office is open Monday through Friday from 8:00 a.m. to 5:00 p.m. We will make every attempt to schedule your appointment as soon as possible. Please advise us if you are experiencing discomfort or having an emergency that requires prompt attention, every attempt will be made to see you that day.

We try to stay on schedule to minimize your waiting and we realize your time is valuable. Because we provide surgical services, adhering to a strict schedule in a surgical facility is not always possible. Treatment of patients on an emergency basis can also cause delays to the regular schedule. We appreciate your understanding.
